We have studied Florence Nightingale and her statistics that drove her to promote simple interventions like fresh and sunshine for sick patients as those correlated to better outcomes. With hospitals merging and acquiring into larger corporations and greater amounts of technology in hospitals, they are developing massive data sets that can be used to make evidence based practice determination. Every time a health care provider provides a medication or checks a blood sugar, the results are recorded automatically in many systems. Assessments by providers are entered directly into the electronic health record. All of this builds data sets that can be used for hospitals to continually refine best practices in caring for patients; however, significant confounding factors exist with regard to comparison. For instance, outcomes can be affected by the patient’s insurance, hospital requirements to treat (non-profit status), skill of the doctor, adherence to therapy and treatment decisions. All of these complicate the picture and would require any study to divide interventions in a blind fashion so that the other factors are washed out. Ultimately, finding ways to use all of this data is a challenge and changing clinical interventions based upon this data is not always straight forward. Retrospective analysis of the data may not be useful in determining which specific interventions should be used moving forward. All that said, the data collected can serve as an opportunity and finding new ways to understand it can elucidate simple interventions.
