Patient had right rib fractures, grade 2 liver laceration, T11 T12 compression fractures, right ulnar shaft fracture and a right pilon fracture. Pain is aggravated by movement and alleviated by rest and medication – WBC 13.8↑ – BUN 4↓ – Hgb 12.6↓ – Hct 37.7↓ – Glucose 123↑ – Sodium 136 ↓ – Grade 2 Lacerated liver – T11, T12 compression Fracture – Multiple right rib fractures The worst-case scenarios that could potentially happen during your shift include:Hemorrhagic Shock: The patient has a grade 2 liver laceration and multiple rib fractures. These injuries can lead to significant internal bleeding. The decreased Hgb and Hct levels indicate that the patient may already be experiencing some degree of blood loss. If not managed promptly, this could lead to hemorrhagic shock, which is life-threatening. Respiratory Complications: The multiple rib fractures could potentially lead to a pneumothorax (collapsed lung) or hemothorax (blood in the chest cavity), both of which can cause severe respiratory distress. Spinal Cord Injury: The T11 and T12 compression fractures could potentially lead to a spinal cord injury. Depending on the extent of the injury, this could result in paralysis or other neurological deficits. Infection: The elevated WBC count suggests that the patient may already have an infection. If not managed properly, this could lead to sepsis, a severe and potentially fatal systemic infection. Renal Failure: The decreased BUN level could indicate a problem with kidney function. If the kidneys are not able to properly filter waste from the blood, this could lead to renal failure.
