My internship placement is as a medical social worker specializing in geriatrics. My program provides wrap around care for Medicare and Medicaid eligible patients that qualify for state long term care assistance. An extremely common ethical concern that we run in to is autonomy and capacity to make decisions. Our client population has a high rate of patients with learning disabilities, chronic health conditions that may impair cognition, and dementia or Alzheimer’s. Disease. Our interdisciplinary team has frequent and long winded discussions about whether a patient has capacity, and disciplines are often in disagreement. Our community has a local initiative called the Older Adult Behavioral Health Initiative. One benefit that this organization offers is trainings and handouts for health care professionals (Oregon Behavioral Health Initiative, 2023). The organization also serves to advocate for older adult issues beyond capacity education – they provide advocates to assist with navigating community resources, case consultation, and ongoing community trainings (2023).
