What comes to mind when you think about performing an initial physical assessment on a new patient? As a nurse you should be assessing your patient from the moment you lay eyes on them. There are so many ways to assess a patient when you initially meet him or her. You will assess their appearance, are they answering questions appropriately, what non-verbals are they displaying, and the list goes on. The first few minutes of meeting a patient are crucial. Your initial meeting sets the stage for an effective interaction, physical assessment, interview, and establishing a trusting relationship. Clear introductions and communication are imperative. This initial meeting and introduction will allow you to reassure your patient and provide comfort as patients can sometimes have feelings of fear and anxiety. Through your assessment and interview process, you will utilize observation, questioning, and listening. What is your patient’s chief complaint? What symptoms are they having? What is their health history? What could be causing these symptoms? What is their perception of health and their current situation? How is their mental and emotional health? Do they have any spiritual or cultural beliefs that you need to know about to better care for them? What diagnostic tests could be beneficial? Your discussion this week will allow you to address and answer some of these important aspects that are crucial in the admission and assessment process.
