Gathers information appropriately for assessment of client that is individualized and holistic. Demonstrates ability to make clinical decisions based on changes in client condition. Student – provide an example Midterm: I was able to demonstrate ability to make clinical decisions by providing emotional support and encouraging the patient to participate in therapeutic activities, such as group therapy and individual counseling scheduled for all the patients. This could help him cope with his traumatic experiences and substance use disorders. Final: 2.5 3. Assesses client for learning readiness and needs to develop and implement an individualized teaching plan. Student – provide an example Midterm: The learning needs of the patients are identified by determining what the patient needs to know or do to manage their mental health condition. This was done by asking the patients about their knowledge of their condition, their understanding of the treatment plan, their skills in managing their condition and their motivation for treatment.
