Nurses working in a small rural hospital have identified low numbers of sustained breastfeeding following discharge from their OB unit. They summarized the evidence but found that with their limited resources within their facility the recommendations would not be feasible. Further, the patient population was predominately poorly educated and of a poor economic background, making implementation even more challenging. The nurses transformed the emerging recommendations to fit their setting and patient population and realized an improvement in breastfeeding rates three and six months post-partum.
