a 57 year old caucasian male presents with a chief complaint of left knee pain that has been ongoing for two days. he describes the pain as constant, with a severity ranging from 6-8 out of 10 on the pain scale. the pain initially began as an ache but has since progressed to a throbbing sensation. the patient denies any radiation oof the pain to the other locations. the left knee is warm and exhibits visible swelling. patient reports experiencing a similar episode of knee pain one year ago. it is pertinent to mention that he has a dietary history of high in purines and recently initiated the use of hydrochlorothiazide. the pain unrelieved by ibuprofen and worsen with ambulation. additionally patient reports swelling and redness in the left great toe metacarpophalangeal joint which has been persisting for one week
