Endocrinology Standardized Clinical Case Study NURS 647 Harold Johnson is a 70-year-old African American male who presents to the clinic for follow-up of Type II diabetes. Chief Complaint: “Not really having any problems. I just want to get established with a new health care provider. You came highly recommended.” HPI: Patient states he has had diabetes for a “long time”. He does not routinely check his blood sugars at home. When he does check, he states they range between 95-130. He would like to have his current medications refilled. H.J. cannot remember his last complete physical examination. He does not routinely go to his eye doctor nor does he remember the last time he had a foot examination. States he generally “feels good” and doesn’t see much of a reason to get “checked” if nothing is wrong. His wife is the one who pushed for the appointment. Past Medical History: Wears bifocals the last 6 years; no history of hypertension, cancers; Has been diagnosed with Type II diabetes 10 years Past Surgical History: Vasectomy at age 35 years; Appendectomy at age 4 years; Left knee arthroscopy in college Family History: Father deceased, age 75 years from MI, history of CAD
