H. Her last INR, 3 weeks ago, was 2.4.  Today her INR is 4.5.  On further questioning, you discover that she was treated three days ago at the local Urgent Care Clinic with sulfamethoxazole/trimethoprim (Bactrim) 800mg/160mg, two times daily for seven days for a urinary tract infection.  She is being treated for seven days rather than three because she had a UTI one month ago. Warfarin and Bactrim are her only medications.
