H. B. is a 60-year-old female who is in your primary care clinic today for increased bruising of her arms and legs. States the bruises appeared suddenly yesterday, and she doesn’t remember an injury. She came in because she is worried due to her warfarin. She is on warfarin 5 mg daily for a deep vein thrombosis. She has been on it for two months and is to be on it for another four months. Her last INR, 3 weeks ago, was 2.4. Today her INR is 4.5. On further questioning, you discover that she was treated three days ago at the local Urgent Care Clinic with sulfamethoxazole/trimethoprim (Bactrim) 800mg/160mg, two times daily for seven days for a urinary tract infection. She is being treated for seven days rather than three because she had a UTI one month ago. Warfarin and Bactrim are her only medications. Â
